PRIVACY POLICY

It is the policy of Designs for Health, Inc. to respect your privacy, and the privacy of all visitors to our website.

We firmly believe that maintaining patient and practitioner privacy plays a vital role in exceeding our customers' expectations. For that reason, we have established this policy statement to help you understand our commitment to privacy protection, and the steps we take to ensure it.

Personal Information
It is Designs for Health's policy to collect only certain personal information, such as names, addresses, telephone/fax numbers, or e-mail addresses, when that information is voluntarily submitted by our visitors. This information will be used only to answer your requests for information, or to better understand your needs, and within our own organization, to improve our services and to better target our informational offerings. Your personal information will be kept strictly confidential and will not be sold, reused, rented, loaned or otherwise disclosed to anyone outside Designs for Health and our necessary service providers.

Additional guidelines related to our use of personal information are as follows:
  1. Premiere PlatformSM patient direct mail pieces will be distributed only after prior approval by the referring practitioner.
  2. A name may be removed from our database simply by contacting Designs for Health at 800-FOR-HEALTH (800-367-4325) , or by sending a request for removal to: Gail Lizotte, General Manager, Designs for Health, Inc., 2 North Road, East Windsor, CT 06088.
  3. If you elect to provide us with additional information (by filling out IRS forms, opinion questionnaires, etc.), we will collect and use this information for internal purposes only.
Aggregated Information
Designs for Health may use your personal information to generate aggregated information that does not allow you to be personally identified or contacted. Aggregated information shows user activity as a whole rather than on an individual basis, and may be used by Designs for Health to provide insight regarding the usage of our services. For example, we might compile personal information in order to determine the percentage of users living in a particular geographic area.

Children
The Designs for Health site is not intended for, nor is it designed to attract, children under the age of 13. We do not collect personally identifiable data from any person we know to be under the age of 13.
